Making Your Appointment
To set up an appointment for an abortion, call Women’s Health Horizons at 315-432-1048, or our toll free number 800-439-1105 between 9:00 am and 4:30 pm Monday through Friday. We can usually schedule you within two to three days of your call. Occassionally, same day appointments are available.
We will need to know when the first day of your last menstrual period was. You are welcome to call us with any questions you may have prior to actually scheduling your appointment.
We participate with most insurance plans with prior verification and proper referrals. If you have insurance please tell us when you call for your appointment. We do not accept Medicaid.
After you have scheduled your appointment, you can download the form below that you will need for your appointment. If you have access to a computer, download your patient forms, fill them out, and bring them with you to your scheduled appointment -- saving you time on the day of your appointment in our office.
| › | You will also be asked not to eat or drink 4 hours before your appointment. |
| › | If you take medication on a regular basis, you may take your medication that day. |
| › | It is always a good idea to wear loose fitting comfortable clothes the day of your appointment. |
| › | It is always recommended that you bring one person with you the day of your appointment, but not necessary. |
| › | However, if you are planning to have the OPTIONAL, but recommended conscious sedation, it is ABSOLUTELY NECESSARY, that you have someone with you to drive you home after your appointment! |
| › | It is also a good idea to bring a sweatshirt and socks, as some people who are waiting in the waiting room for some period of time, start to feel very cold. |

Confidentiality
Our staff will not discuss your appointment with anyone outside our office. You can be assured of our complete confidentiality. We will require a phone number, in case we need to change your appointment, but we will only speak directly with you. Also, you will be called from the waiting room using your first name only.
Consent
Abortion is legal for women of all ages and parental or spousal consent is not required in New York State.
Fees and Payment
Payment is expected at the time of your appointment. We accept cash, money orders, Visa, MasterCard or Discover. Personal checks are not accepted.

A VERY IMPORTANT NOTE REGARDING CONSCIOUS SEDATION! IF YOU ARE PLANNING TO HAVE THIS, IT IS ABSOLUTELY NECESSARY THAT YOU HAVE SOMEONE TO DRIVE YOU HOME! YOU WILL NOT BE “ASLEEP” FOR THE PROCEDURE, YOU WILL BE CONSCIOUS, AND TALKING WITH THE NURSE AND DOCTOR IN THE ROOM. MOST PEOPLE ARE VERY COMFORTABLE, AND MOST PEOPLE HAVE LITTLE OR NO MEMORY OF THE PROCEDURE, HOWEVER EVERYONE IS DIFFERENT, AND WE ARE VERY CAREFUL WITH THE MEDICATIONS WE USE, AND THE AMOUNTS. NOT ALL WOMEN’S EXPERIENCES ARE THE SAME.
If you choose to have your follow up appointment in our office after your surgery, there is a $70.00 fee that is payable at the time of your visit. We will accept cash, MasterCard, Visa, or Discover for payment of this visit. Often times, insurance can be used for four follow-up appointments.

You will continue on to have a complete physical examination by the doctor. After your exam, you will speak with the doctor in his office again, and be given your medication, along with your instructions.
Remember, you must be seen in 2 weeks, for a follow up sonogram, and another examination by the doctor to make sure that the pregnancy has been completely expelled. This visit is included in the original payment.
